But where do I get airline or ferry staff in the middle of Northern Ireland and Republic of Ireland?

Maybe it's illegal to cross using this type of transport.
You are not travelling by airline or ferry so you do not need to present the form to airline or ferry staff.

They did not mention bus or train because bus and train staff will not ask you for the form.

The majority of people travelling by bus or train do not need to fill out a form as they will have been on the island of Ireland for 14 days.

If you are asked for the form, it will probably be by the Irish police when you disembark the bus in Dublin. But it is unlikely you will be asked at all.
