Flew on AS1273 PVR-SFO just under a month ago, mid-Oct. Flight was at 7PM, we arrived at 5PM but didn't need to.

Zero people line for the health check, walked right up. Had to sign/date a health declaration form then they check that your COVID tests results are negative, the dates, and your name matches. Took about 90 seconds.

When we went, the health check was just a 6ft folding table, manned by two very friendly, young Mexican gentlemen. They were very cheerful and helpful. The table was located and visible as soon as your enter inside, before you would get in line for the airline checkin counters. When you checkin/drop your bags at the Alaska counter, you give them the health form from the health check table beforehand.
