I asked for a confirmation I could use for the Thailand Pass (I had booked through FHR and paid separately for Test & Go), and my hotel sent me a confirmation that clearly spells all the pieces out (and has both our names). I think that there is some kind of standard confirmation called SHABA they should be able to provide.
