It's been about 4 years since our visit to Naples but I recall they used set pricing... about 20 Euro from airport to hotel, perhaps? There should be a schedule posted in the cab and s/he should inform you what it is. I feel no need to tip under those circumstances, but sometimes I do.
The most unsettling part was being stopped in airport arrivals by plain clothes people (no i.d.) asking about how much cash we were carrying -- apparently they are looking for drug smugglers and they target well-dressed people...
