At some airports, this has been a thing -- but IME those have mostly been non-lounge showers at the airports. The first time I encountered such an arrangement was at SYD, but that wasn't part of a lounge. Sort of like some European campground showers, except the European ones required paying for shower/water time and didn't come with towels (unless paying for towel service on top of shower); the SYD shower was free, but no towels included. Ski housing rentals in some parts of Europe don't include towels with the hired room/accommodation unless paying a supplement for linen service for the accommodation.
Seem so weird not to supply towels with a shower service. It's like going to a restaurant, and flatware costs extra.
