Also check if the actual provider you received your shots from (not just Connecticut) uses MyChart (Epic) or Cerner electronic systems. Some co-workers of mine found that while their state isn't a SMART Health Card issuer, the provider they got their shots from uses Epic which can produce a QR code acceptable to Singapore for VTL purposes.
