Upgrading HA w/Miles

Yes, from any/lowest Main Cabin Fare but not from Main Cabin Basic. Note: the miles upgrade will need to be processed by an agent over the phone. There is a booking fee of $25 if you purchase your Main Cabin coach ticket over phone prior to having the agent process the upgrade. We have found that sometimes calling to reconfirm upgrade availability and then completing booking/upgrade with same agent is worth the $25 fee per ticket on high demand/low inventory routes like HNL/JFK. YMMV

As you noted there are two levels of N American mileage upgrades Saver/25,000 one way and Flex 50,000 one way, searchable on ExpertFlyer. Saver awards tend to be something of a unicorn lately.
