That gate agent will be praying you delete the details of this flight. If Delta reads it, there's a LARGE chance they're fired. Probably guaranteed if this wasn't crazy COVID times, but perhaps grace will be given with what they have gone through.
The GA can and should write it up themselves. It's a semi-'get out of jail card' if they self report the problem.
Systems and procedures fail and if there were multiple persons working the gate, which is a normal occurrence, a combination of events, and failed checks or conrols might be the cause.
