I don't think travel insurance is the vehicle for this. I think it should be relatively easy in Ecuador, but otherwise an outfit called QuckMD wrote up a certificate of immunity after a 1 minute phone call. Then you are cleared by a licensed US based MD, so no questions should be asked. Costs $75. Good line of business!
