So from a LAX check-in agent perspective the arrival minus 72 hrs. is what they would enforce (if they were to enforce anything at all with regards to Italy). You have a ticket from LAX to Milan and as far as they're concerned you're connecting right through LHR (and not entering the UK) no different than if you had a 2 hr connection.

Again all of the above only matters if they can check you in all the way to MXP, at which point they would want to see the required docs for Italy.

If they only check you in to LHR (most likely) then the required docs for Italy would never come into play.
