Very urgent question. Apologies in advance for the rush and lack of time to research. A US citizen on OCI has a flight from BLR-DOH-IAH tomorrow and has just got the RT-PCR negative test report (after fighting battles to fix serious typos). The QR website is little skimpy on details (unlike that of UA for non-stop flights from India to the US).

1) already downloaded and filled out "PASSENGER DISCLOSURE AND ATTESTATION to the USA" (OMB Control No.: 0920-1318)
2) already downloaded and filled out "PUBLIC HEALTH COVID-19 PASSENGER SELF DECLARATION FORM"
3) already downloaded the "Qatar PCR Test Passenger Consent Form"

QUESTIONS:
1) What are the next steps? Anything else I am missing?
2) Please clarify if "Air Suvidha" is ONLY for India bound passengers, and not for those leaving India bound to the USA. Please clarify.
