Got the bonus, then huge refund….what to do?

So my wife hs the Quest card, and I just realized something fairly worrisome on her account that I manage.
Basically, she received the entire 100k bonus (80k for $5k within the first 3 months + 20k for $5k more within 6 months). The problem? There is only $1,700 of spend on the account and we are on month 4.
How did that happen? I had what I thought was the perfect plan. I used Plastiq to cut a check for our 6-month house rental for $10,500 (we are WFH nomads). It was all above board - I sent in the lease and everything. However, Plastiq refunded the amount because they consider 6 months too short term, and their ToS exclude short term rentals. The $10,500 was refunded to her card like 5 days later. However, the very first statement that reflects the charge and refund also reflects the 100k point transfer to United.
That was four months ago, and I just realized the totality tonight. You may be skeptical, which is totally fair. I would say it's a combination of both Chase and United reflecting we had met the threshold, moving, general disorganization, and having a lot of credit cards. I assure you there was no ill-intent.
So that leaves me wondering what to do next. I can think of a few options:
1.) Do nothing, leave it be, and just hope/assume nothing happens
2.) Quietly spend the $10k in the next 2 months (We're on month 4), so we will have officially spent the entire amount (though we will not have met the official first 3 month requirement). My thought here is if they do look into it, a reasonable employee may see that and judge that we are not abusing the system and move on.
3.) I could call Chase and ask what they think we should do, though the downside would be drawing attention to the account.
I googled around and most articles indicate that Chase does not claw back bonuses, but this seems particularly egregious. Any help would be appreciated.
