Too late to help the OP, but in case anyone else is looking, https://www.passporthealthusa.com has offices in many parts of the US and has the ability to give a PCR test with wet ink signature, passport number, etc.. You will have to go in and take the test, and then go back the next day to pick up the results. Not cheap, but reliable.
