I've never seen this, but I suppose there may be some outliers where this happens. My understanding is the guest policy is going to be based on the membership. For the plat, there is 1 guest allowed.
Where I have seen a difference is comp. access for children - most of the lounges I have noticed allow free access for children under 2 (when accompanied by a member, of course), so they don't count as the guest, but I've seen others, I think mostly 5?

That is my experience as well (applied in July). My plat card was sent out day after approval, IIRC, via UPS overnight, and I was given 'instant access' to put in my iPhone wallet. My wife's gold was sent via USPS, and took about a week, maybe a bit longer.
