If there is a major change to your reservation (new flight numbers, routing change, etc) then the ticket needs to be reissued and you get a new ticket number. If it was a 'parked' ticket, then there is a manual process where someone needs to dig out the original ticket and exchange it for a new ticket. This process takes time and if the departure is not imminent, it might be lingering in a queue waiting for the many back office agents to get to it.

Upgrades on a fixed price do not change the underlying ticket. Here the booking and service class is changed in the reservation and an EMD is created for the amount collected from you. The system attaches the EMD to the booking and a reference is made to indicate that the change has been accounted for. Upgrade (myoffer or fixed price) cannot be done until the underlying ticket is in line with the reservation. The reason is simple: if the back office gets to the ticket and sees a different class of service and booking class then the underlying ticket will not be reissued becuase the details of the fare paid and the booked class don't match.

Bottom line is that since the trip is in October, nothing much will happen to the ticket in the interim. FFP details can be added a few days before departure after the ticket is finally reissued. You could call the german phone numbers during german business hours and select German as language. With some luck you will reach an agent in IST or BER who will reissue the ticket manually on the spot. See experiences here: Ticket not re-issued 3 weeks after change
