For a normal reward booking made online and with no subsequent changes it would be very unusual as the eticket normally issues straight away.

It may be there was a change and it was a free one (BWC/Invol) but no ticket was issued, or that payment was required but never taken and so a ticket was never issued, or that a FTV was used but the ticket was never issued.
