As an alternative to the expensive Doctors on Call testing ... (if that ends up working for Canada)
Take a look at Clinical Labs of Hawaii, their test is currently $120. (It was $150 when we did it for inter-island travel in January.)
Go in the morning, as they send the test over to their lab in Oahu. They have a location in Kihei. You can ask them what time the samples get picked up to plan accordingly.
