LAX-CDG-LIS COVID procedure Questions

I'm flying in J LAX-CDG-LIS in early August, and returning the same route 2 weeks later. I'm a fully vaccinated US citizen with vaccination card. As I understand it, France does not require a negative COVID test for entry, but Portugal does. I have the following questions:

1) Is my original vaccination card sufficient for boarding/entry to CDG or do I need to use some type of app to upload a picture into?
2) AF, per their site, requires a "surgical" (not cloth) mask during transit. I take it, that this means the thin (usually blue) disposable paper mask that you can get boxes of on Amazon that expand like an accordion?
3) Since I'm transiting to Lisbon, will AF accept the PROCTORED Abbott BinaxNOW at-home COVID test results for entry into LIS and for returning to the USA? CDC says the PROCTORED BinaxNOW is acceptable for US entry.
