Yes, Amex PCRP works when CDW is not declined. A good example of this is Italy where by law, basic CDW (and theft and liability) must be included (and thus cannot be declined).
"Super CDW" which waives the excess (a.k.a. deductible) is an optional add-on. You can, however, rely on Amex PCRP instead of that, and if you're in an accident, Amex PCRP will cover the excess/deductible that isn't included in the rental contract.
In short, Amex PCRP does not have any country exclusions that we're aware of and does not require you to decline any included CDW in order to be valid.
