In the past standard Amex rental car coverage had language to that effect (allowing CDW to be accepted), but current standard coverage contracts no longer include that language.
If you don't decline CDW, then you are paying for AMEX Premium Rental coverage for no reason as car rental should be "waiving" damages to the vehicle due to CDW/LDW and not send you a damage claim for AMEX to cover.

I believe you can call AMEX and have them credit back any premium paid when you provide them a copy of rental agreement stating CDW/LDW was included.
