The OP never answered our question about their eligibility for MMP which makes me think that the FDA asked for the form and they didn’t have one so hence the price increase.
If the hotel deems the traveler is not eligible for the booking, the reservation with the low rate would be cancelled. Then OP has to book a brand new room at that spot and time. I think that was what happened, just my guess.
I sensed some important details are clearly missing here. Common sense does not prevail....
