I had a trip booked in first for two of us with the Reserve Companion voucher for April but we couldn't go, due to COVID issues (everyone is fine now). I believe typically when you cancel a companion certificate booking, you keep the paid-in credit but aren't able to reuse the companion certificate itself. If you have proof that you were COVID-positive or similar extenuating circumstances, it couldn't hurt to ask Delta for an exception, but yes you would have to call and explain the situation, it wouldn't happen automatically. (I'm assuming you're talking about April 2021, if this was something that happened in April 2020 it may be too late to do anything about it.)
