Here's how to do it
  1. Call Flying Blue and book the reservation with a hold. You need to tell them you are going to transfer miles from AMEX Membership Rewards. (I suppose they could do this for another program but I don't know.)
  2. They will give you a PNR and tell you how much time you have on hold and how many miles you need.
  3. Call Membership Rewards and transfer the miles. It worked instantly.
  4. Call Flying Blue back with the PNR. They will see that you have enough miles, deduct them and finalize the booking.
