I'm wondering the need for this - like international driver's permits don't map to local driving registries, my vaccination records shouldn't have to move local registries when I am travelling.

I think for permanent moves, I might need a transition (or if it is easier, I'll just re-do the vaccine). Much like I need my HK DL moved to NSW DL if I move back to Sydney.
