I pat down alot of people everyday because they dont want to go through the WTMD. It is their option to exercise this right due to any reason but the most common reason is medical. They dont have to inform us of a reason for the request because it can be embarrassing for them. A word or advice though, if you opt to try this approach and are offended by the limited pat down of secondary screening, dont go for the full pat down. During a full pat down you have to remove EVERYTHING from your pockets and your shoes and belt.
I have worn orthotics since 1995. I have yet to come across a TSA employese who has not inisted that I remove my shoes even upon being told that I wear orthotics. Lot of the niceties that are provided, it seems, are well guarded secrets.
