For the timing of the test, be really careful depending on the requirement of your destination or transit point. Do it at the earliest possible time depending on the requirement, and do not hesitate to tell them when you need the results. ​​​​​
Just to report how it works now.
One must book an appointment online at https://zanzibarcovidtesting.co.tz/app/home
One can pay online or in situ. If the latter you can even mix different currencies (we used TZS and EUR and forex rates seemed fair like 2750).
If the process was really straight forward, hygiene rules are a joke, half of the staff not wearing mask, (cute) kittens and cats freely roaming inside the building, no separate toilet (no paper btw) for female or male ( in a muslim island???) and the guy taking the swab looked nowhere as a health professional, wearing gardening boots and just gloves and face mask no gown nor shield.

Test done Wednesday morning at 10h30. No need to collect the result at the center but now sent to email ( on Friday morning at 6am).
The test must be printed because it must be stamped by officials at airport. If not printed, at DAR there is a stationery shop where it can be done. Don't forget to mention where you did the test in case the city is different than the airport (= test done in Stone Town but we departed from DAR) otherwise they won't find in the system.
