Let me try this again. I have a FFC for $200 and my companion has a FFC for $200 each from separately cancelled PNRs. I can book a new PNR for $200 each ($400 total) for two of us using both FFCs. We are on the same PNR. The only issue is you need to have same fare, and same fare class, available.

Let me try yet again in verbose form.

1) I booked myself on PNR AAA111 for $200;
2) My companion booked later PNR BBB222 for $200;
3) We canceled AAA111 and BBB222 and received two FFCs;
4) We booked PNR CCC333 for $200 each passenger paid with FFC AAA111 and BBB222.

Same fare and fare class must be available for both passengers.
jsloan listed a manual process as well. It is better but you need to convince someone to process it manually.

It is complicated and many newer agents probably don't even know what you are talking about.
