One reason for this is the fare / fare class / fare basis for PNR must be common / the same for everyone on the ticket. The underlying industry systems are no capable of having a common PNR with different travel details. Not likely to see that anytime soon.
Just because something is not possible with a current system, and is outdated across the industry, isn't a good reason for it to persist. Those are excuses. A redesign can certainly be done to accommodate this feature, but no airlines are willing to put in the time or money to do so. Just say it man, or I'll do it for you:
enhancing the current system to accommodate the merge of a spouses ticket into one reservation cost more time and money than UA is willing to expend to pursue this effort.
It's anti-consumer and it should be addressed.
