Fair enough, the interpretation of courtesy is a very subjective area! I don't personally find the use of my first name - or even hey - discourteous ('I draw the line at 'YOU'

) and I would find it mildly uncomfortable if I was constantly addressed as 'Mr...'
Or, put another way, I would consider someone more courteous if they delivered an exceptional service using familiar terms, having taken the time to build some kind of rapport, than a tokenistic service delivered using a more consistent and formal type of address.
