Indeed TP could have, but did not, deny your refund. Perhaps a fare rule or other. But, that really is not relevant. Maybe:
1. When my ticket was cancelled, TP offered a refund and I accepted.
2. TP provided a # for the refund, but never paid.
3. Multiple attempts to collect the refund have failed as well.
Make certain to document each assertion with the relevant documents.
