When you have completed the health questionaire, you will get the QR code sent to you by e mail. Print it out. But it won't have additional important text below it, so sign into the Hawaii website, and get a screen capture of the QR code with the 6 or 8 lines of critical text below. That is what hotels, car rentals, golf courses and other want to see.
