When it has happened since then I have been able to fix it myself or get it fixed and the counter at the airport. Yes, it is an AS IT issue IMHO.
There was a period of time when the KTN from a profile was not populated into the PNR. You can always confirm/add your KTN at the start of the OLCI process.
