The $20 fee is for Hawaii documentation which they email you a link for. I told them I had no insurance and paid the $20. The way they work currently you need to prepay the $20 when you make the appointment. I doubt insurance will reimburse it but I didn’t try.
Thanks, I don't mind paying the $20. I am just worried that my insurance provide won't cover the cost of testing and I get stuck with a larger fee than the $20 they are advertising on their website.
