Happened to me once.
You need to go to the store with your purchase receipt and the inactivated card and show it to the manager. They are able to reissue you a new card.
What happened was that the store never activated the card and that was reflected on the receipt.
I went back to store and they are adamant that it is a card issue and not their fault. Now Staples CS has put a ticket with Blackhawk to figure it out. Apparently Blackhawk wont talk directly to end customers to fix it.
