I have had an issue once, in Russia. First they did not want to accept two guest for the room as the booking clearly said 1. Then in the morning they wanted to charge the breakfast for person number two, despite the room rate clearly said includes breakfast for two people. Reason? The booking only said 1 guest. After some discussion the hotel did capitulate, and second morning all was without remarks.

So there might be the odd case here or there, but in the US I doubt any odd cases would ever occur.
