I am in USA on H1B Visa. I am going to apply using VFS.
Have couple of ques on VFS documentation checklist. If someone has submitted application via VFS, can you please help answer these ques.

1. VFS Checklist asks for two photos: One photo goes on govt form. This is fine. But, other photo, at one place it says clip to the form; but in next line it says paste photo on change of appearance form. But, I have not received any such form. So, do i clip the photo on the govt form or somehow get this appearance form?

2. Do we need to notarize the proof of address?
