I have used multiple systems to get the print. One is giving me 3 pages (the way you said) and another system 2 pages. I have used the 2pages one and submitted to VFS.
I have used their FEDEX, don't want to deal with my own prepaid labels. I know theirs bit expensive but it's not worth the headache.
Checklist from VFS is out of the whack, but followed what it says and submitted.
I have same issue as yours with VISA. I have used only passport info on both fields and submitted GE application on TTP website.
