Great discussion, I was not actually expecting this much banter, I have to laugh at the situation. A few answers:

I did have a US Air number, you made me dig for that one . No luck.
I have not signed up for any credit cards or other associated accounts, good question.
I do have plenty of documentation showing my number, name and corresponding info.

When I get a chance, I am going to call them back and try again. I will ask them if I can submit documentation and see if they can resurrect the account . MC
