I couldn't find anything about the format of the PCR test result on the KLM website or on the Dutch government websites, but an email I just received from KLM about an upcoming flight indeed says this:

"PCR test for passengers travelling to Amsterdam
The Dutch government requires that if you arrive from high risk country that is not on the safe countries of origin list, you need to show a paper copy of a negative COVID-19 test result not older than 72 hours prior to arrival and a paper copy of a signed declaration. This also applies to transfer passengers."

Meanwhile, Shiphol airport explicitly says (https://www.schiphol.nl/en/messages/...virus-update):

"...you are required to show your airline a declaration that you tested negative for COVID-19. The declaration must be brought with you throughout your entire journey. This can also be done digitally."
