I have figured out previously a few reasons that B&H does not show up

Less than 48 hours before flight
Use of the 5% link
Use of an E-cert
Not authorized for B&H feature
Award tickets

I had not tried the internet fares that Jamester mentioned, but that makes sense. However these is still some other sequence of events that won't alow B&H.
