WOW just wow how Avianca/LifeMiles believes that other people's money grows on trees! These are not isolated incidents, we have heard so many sad stories like this! At a minimum, this is incompetence and dereliction of responsibility and obligations by this carrier. How did Star Alliance accepted this airline into its fold is beyond me!
