I successfully used this offer with United and also with American. In both cases, I clicked on the offer that appeared on the payment page of the reservation. I was immediately approved and given a temporary credit card number and a link to return to the reservation, where I successfully paid. It took about 2 billing cycles for the credit to show up. I think you have to use the link on the reservation to get the temporary # or else you will need to wait for real card.
