There is no such thing as international transit in the USA, you have to enter the USA via landside and back in again, though in the case of MIA there is a route which minimises the impact of that. You will probably need the relevant ESTA or Visa if you are an alien, there are only a few execptions. For the lounges, that seems a purely AA issue so I'm moving it on to the AA forum.
