I understand there's a lot going on but that's just obscene.

Has anyone had any luck getting their cash refunds to be processed faster? At this point, it just feels like they're stalling and either hope we forget about it or they just go completely belly up and don't have to pay out refunds to their customers.

Plain and simple, your only options are to either wait at least 5 months or open a dispute with your credit card company. For all we know Avianca and Lifemiles may go under so I would recommend contacting your credit card asap.

Note that Lifemiles hasn't actually processed any refunds since April.
