The actual wording from the standard guidelines is this, so if your outbound is NCL-LHR-MAD then is it 120 mins from origin (NCL) to destination (MAD).

If connecting flights are on the same ticket, then 120 minutes is the schedule change from point of origin to the point of destination, including the time to transfer
Tbh with the reduced frequencies of flights atm I would be surprised if there is much scope for any schedule change for one flight on a connecting itinerary that doesn't result in over 120 mins change from origin to destination.
