Yes. All arrivals from outside the UK/JER/CTA need to fill in the form even if you are coming from a country exempt from self isolation.
There is a page which relates to exemptions in the form when you complete it. When I did it the other week there was no specific check box for exempt countries so I ticket other and noted in the free text box that I was coming from Spain which was exempt from self isolation. Hopefully they will have put in a specific check box for exempt countries now.
