I live outside EU and had not paid my entire trip with AMEX, but I had paid everything inside for my EU side trip in/from Spain, tickets, hotels, food with AMEX. Unfortunately KLM canceled my onward flight from Spain to AMS (from where i would return to my country of residence), without offering an alternative. Thus we had to arrange travel to AMS at much higher cost than what KLM would refund. Also agree with your communication gripe. We pay premium fees, but only get to have interactions by phone with ever so kind (in Holland at least) agents, who in some cases are not very experienced and totally ill equipped when a situation is complex.
Ugh. That stinks.

Another problem with Axa agents... sometimes you get really knowledgeable ones and sometimes you get ones that have no idea what they are talking about.

Actually I am in the process of planning a multi-segment trip (US/Mexico/Caribbean) and I called to get clarification.

The first agent I spoke with said the only baggage or flight delay covered was my departure from France (WRONG... I said thank you and hung up).
Called back and spoke with another agent who was clearly very experienced. What I learned was the following--- and I have read their policy VERY carefully.

1) Wherever your card is issued, you have to show that you departed from that country and returned to that country. For example, in my case, CDG-JFK round trip. That entitles me to medical coverage for any additional flights (JFK-MEX-LAX-YVR, whatever) I might take (max 90 day trip) EVEN IF paid with a different credit card.

2) IF however, you want the travel coverage (cancellation, delay, baggage) then each segment must be paid with an American Express card. That makes more sense to me.
