hfly, someone could take 30 or 50 or 100 flights and if they were all booked in M,N,V,etc, end up with 0 in tier points. I am not familiar with any escape clause to avoid this.
If BA really wanted to make some changes to the US-EC program (customer service aside), they could do two things IMO: 1) Tier points for all flights and 2) Allow upgrades from restricted fares, even at a higher miles cost than full Y upgrades like the QF system. Then, I would be content. Well, at least for a little while.
