Delta is very good at giving waivers (to waive change fees ) if you have to change or delay travel due to medical situations. The original ticket will need to be rebooked and any additional air fare will still need to be rebooked and traveled by one year from the original ticket purchased. Obtaining a complete refund is less likely.
