I get this effect all the time using an iPhone and official Apple cable on trains around the UK (using the cable straight into a USB port as well as using the official mains adapter). The only way to use the phone effectively is to unplug it. Apparant high speed charging stations from Huwai at RUH Airport didn't have the issue, so not sure about the voltage answer, as surely high speed has more?
